This form is for Law students who would like to request an attestation letter from the SAO for a letter that is not already available via Minerva or any other source. This is聽not聽the form to use if you are requesting a Bar/Licensing-related attestation; the Bar Attestation form can be found here.听BEFORE submitting, please FIRST check to see if the letter you need is available on Minerva.
